All new patients are required to complete registration formalities and are offered an appointment with a nurse to enable the surgery to make a full assessment of your medical needs. On registering with the practice you may wish to express a preference of which doctor you would like to be registered with. Please let the receptionist know and we will make the necessary arrangements.
We ask for information about you so that you can receive the best possible care and treatment. We keep this information together with details of your care to ensure that your doctor, nurse or health care professional has accurate and up to date information.
There are such times when we have to pass on information to other people such as hospitals, social services, primary care trust etc. This is always done confidentially.
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
